Patient-Centered Medical Home is a new way of practicing medicine. At Park Avenue Medical, your physician works with a team of other health care professionals and helps you and your family members actively participate in your own health. As your providers, we ensure that you receive easily accessible, continuous and coordinated care for all your medical needs, especially between medical visits. Park Avenue Medical is currently seeking both accreditation and NCQA PCMH recognition.

The Patient Satisfaction Survey is a short, easy questionnaire that provides us with information and insight on our patients' view of the services we provide. Our goal is to use survey results to design and track quality improvement over time, as well using them to help us identify ways of improving the practice.
